Mission – SLI supports the efforts of individuals with diverse abilities to achieve independence and actively participate in the community.
Services & Programs
SLI provides a lifetime of services aimed at supporting individuals with intellectual disabilities. We can serve children at the age of five with our Targeted Case Management program and adults as early as 18 in our Community Integration Program and Community Living.
Community Living includes residential services or independent services. Residential services offer community-based living options in SLI homes that include support to meet the persons served needs, 24-hour on-site support. Independent living services offer the persons served the ability to manage most aspects of day-to-day life independently, to live independently with of the assistance of staff support as needed and emphasis on money management and maintaining a safe and healthy environment.
Community Integration Program is a day service program that is focused on life-enhancing activities, active learning and community involvement. This program is designed to keep people engaged throughout the day with a variety of activities and learning experiences to choose from.
Targeted Case Management is providing assessment to determine service needs, designing a Person Centered Support Plan that meets the persons served needs, what they want and obtaining benefits and services for them.
